The information here is based on a manual that the Coalition for Accessible Family Health History Tools is developing. Two sections are currently available to help get a family health history initiative off the ground:

The manual will supply a comprehensive set of questions that will guide you through the process of making family health history tools accessible to your community. Plus, you will not be left to answer the questions alone. For each question, the manual will help you answer:

  • Why is this question important?
  • How do I answer this question?
  • What resources are available?


The information included here and in the manual should not be limiting. Feel free to supplement these guidelines with information and action from your personal experience, as it can only enhance the value and effectiveness of the initiative. Furthermore, use your experience to improve the information offered here so that others can benefit from your successes.
